What Happens (Spoilers)

The story kicks off in 2026 with Detective Amanda Beck finding a boy's body, his head covered with a shirt, while searching for another missing child, Neil Spencer. This chilling scene looks exactly like the work of the "Whisper Man," Frank Carter, a serial killer from the 1990s who whispered to his victims and tormented their families. He was caught by Detective Peter Willis. Amanda goes to the now-retired Peter for help, but he refuses to talk to Frank, who will only speak to him. Soon, a recording of the Whisper Man speaking to Neil is found.

Meanwhile, novelist Tom Kennedy, who recently lost his wife, moves to her hometown of Garretton with his young son, Jake, hoping for a fresh start. Jake has an imaginary friend, a girl in a blue sweater, and struggles to make new friends after his mother's death. Tom tells Jake about his own childhood imaginary friend, Mr. Night, who would visit him and kiss his forehead before bed. One night, Tom catches Jake reciting the Whisper Man rhyme at the front door, and a hand reaches through the letterbox, but no one is outside. The next day, Tom finds a man in his basement who then flees.

That night, a drunk Tom loses his temper with Jake when his son says his imaginary friend is scared of "the boy in the floor," telling him to grow up. Soon after, Jake is abducted. Desperate, Tom contacts his estranged father, Peter, for help. Peter joins Amanda's investigation. Tom then learns that the previous tenant of his house, Dominic Barnett, was Frank Carter's accomplice. After Neil's body is found, Peter finally agrees to meet Frank. Frank denies involvement in the new murders and taunts Peter, suggesting Tom's detective novels are just an attempt to be like Peter, given their difficult relationship.

While looking through Frank's prison visitors, Peter identifies Norman Collins, who collects serial-killer items but has alibis for the abductions. Amanda and Peter still suspect he's been in touch with Frank. A recording of Jake is then delivered, where he laments that his father didn't believe him. Peter later tells Tom that he was absent from his life due to his alcoholism, caused by his police work, but that he did visit Tom at night and kissed his forehead, revealing that Peter was indeed Mr. Night.

With time running out, Tom follows Norman and recognizes him as the man from his basement. Tom and Peter search the basement and discover "the boy in the floor," which turns out to be the skeletal remains of Frank's fifth victim, along with Barnett's remains. Amanda and Peter confront Norman, establishing that he introduced the copycat killer to Frank and murdered Barnett after Barnett mocked him. Norman then kills himself to avoid prison.

Recalling Frank's earlier remarks, Peter realizes the killer is Frank's son, Francis Jr., who is trying to be like his absent father. Amanda visits the home of Dr. Sharmatz, Francis Jr.'s childhood counselor. She suspects Francis Jr. is there and finds Sharmatz's decomposing body upstairs. Jake's imaginary friend tells him to scream to alert Amanda, but before she can reach him, Francis Jr. stabs her, grabs Jake, and escapes to Tom's house. Meanwhile, Peter realizes Frank covered his victims' faces so he could imagine strangling his own son. He offers to bring Francis Jr. to Frank in exchange for information about their plan to kill Jake in Tom's basement. Tom and Peter arrive at the house, confront Francis Jr., and rescue Jake, but Peter is fatally stabbed. Tom and Peter reconcile before Peter dies. Francis Jr. chases Jake into the attic, where he falls through a damaged floorboard and is incapacitated.

Some time later, Amanda receives a letter from Frank thanking her and Peter for their actions. In prison, Frank is escorted into Francis Jr.'s cell and strangles him. At home, Jake asks to play with other children, showing he's overcoming his trauma. While looking through photos of his wife, Tom discovers one of her as a young girl wearing a blue sweater, revealing she was Jake's imaginary friend.
