What Happens (Spoilers)

The movie kicks off with a bang when teenager Tuesday Campbell is attacked by Ghostface while watching "Horror Movie," an in-universe spoof of "Scary Movie." Her older half-sister, Sara, who’s dealing with drug addiction, and Sara's boyfriend, Jack Kirsch, hear about Tuesday’s hospitalization. They immediately suspect this attack is just like the killing spree by the original Ghostface, Doofy Gilmore, back in 2000. They try to get help from Sara's estranged mother, Cindy Campbell, but Cindy has become a paranoid recluse and refuses, believing the killer is trying to lure her out. This understandably upsets Sara.

Meanwhile, Tuesday's high school friends – Brenda Meeks's children Brad and Dei, Brad's girlfriend Elle, and town sheriff Greg Phillippe's transgender son Jess – also learn about the attack. Soon, Brenda's stoner brother Shorty joins the group. We also see Brenda, now calling herself "Ma," attending church with her husband Ray Wilkins, who claims he’s renounced his homosexuality.

Sara and Jack are later attacked by Ghostface at the hospital but manage to escape. They decide to visit Doofy, who has reverted back to his mentally disabled persona. Doofy reluctantly agrees to help, and the trio heads to Brenda's house. Cindy, having a change of heart, also arrives. Brenda, ever the theorist, starts piecing things together, concluding that Ghostface is taking the "rebootquel" approach – bringing back old characters while introducing new ones. This means all the original characters are now in danger.

The body count rises quickly. Ghostface murders Greg and Jess. Reporter Gail Hailstorm, looking for a scoop, reconnects with her former lover Doofy and trades a handjob for information. Ghostface attacks Tuesday again at the hospital, but Sara and Doofy rush to her rescue. Tragically, Doofy is killed, but Sara gets Tuesday out, and they are saved by Cindy and Brenda. That night, Shorty smokes too much weed and Ghostface hypnotizes him, leading to a bizarre KPop Demon Hunters-style animated sequence.

The chaos continues as Shorty hosts a livestream where Ghostface murders celebrity guest Kai Cenat. Ghostface also infiltrates an experimental procedure Gail undertakes to look young again, injecting her with "The Stuff" before killing her alternate self. Dei meets a grim end on the subway, stabbed to death by Ghostface and several passengers who are fed up with liberal perspectives. While Brenda hosts a Halloween costume party, Brad is killed in the garden. Cindy, in a desperate move, steals a car and breaks into Brenda's house, fighting a wave of Ghostfaces John Wick style.

Finally, Ghostface reveals herself as Elle, with Jack as her accomplice. They explain their motive: they want to take over the "Scary Movie" franchise from the legacy characters. However, their reign is short-lived. Four more Ghostfaces appear and kill them! These new killers unmask themselves as Anthony Anderson and Shaquille O'Neal, accompanied by Shorty and Ray. They reveal they planned the entire ending plot twist. But the betrayals don't stop there: Anthony and Shaquille are then killed by Shorty and Ray, who harbor a grudge against Anthony, Shaquille, Cindy, and Brenda for participating in "Scary Movie 3" and "Scary Movie 4" without them.
