The story kicks off in 2004 with a failed robbery in Ludendorff, North Yankton, involving Michael Townley, Trevor Philips, and Brad Snider. Michael is presumed dead, but fast forward nine years, and he’s living a quiet life in Los Santos under the name Michael De Santa, thanks to a secret deal with FIB agent Dave Norton. Michael meets Franklin Clinton, a young gangbanger working for a corrupt car salesman, when Franklin tries to repossess Michael's son’s car. They become friends, and when Michael catches his wife with her tennis coach, he and Franklin chase the coach to a mansion, damaging it in anger. The owner, drug lord Martin Madrazo, demands payment, forcing Michael back into crime with Franklin as his accomplice. With help from Michael’s old friend, the disabled hacker Lester Crest, they pull off a jewellery store heist to pay off the debt.

Meanwhile, Trevor, who's been living in poverty in Blaine County, hears about the heist and recognizes Michael's style. He believed the FIB killed Michael years ago. Trevor tracks down Michael, forcing a reluctant reunion and bringing chaos back into Michael's life. As things spiral, Michael's family leaves him because of his criminal activities. He later becomes a movie producer, clashing with billionaire Devin Weston, who tries to shut down the studio. Michael thwarts Devin’s efforts, accidentally killing his lawyer, which makes Devin vow revenge. Franklin also has his own troubles, repeatedly having to rescue his friend Lamar Davis from rival gangbanger Harold "Stretch" Joseph, who keeps trying to kill Lamar.

At the same time, Trevor is busy trying to take over various black markets in Blaine County, fighting against the Lost outlaw motorcycle club, Latino street gangs, rival meth dealers, the private military company Merryweather, and triad kingpin Wei Cheng. Because Michael broke his agreement with Dave by committing robberies again, Dave and his superior, Steve Haines, force Michael, Franklin, and Trevor to work for them. They perform operations to undermine the International Affairs Agency (IAA), stealing funds and an experimental nerve agent. As Steve faces more scrutiny, he forces Michael and Franklin to break into the FIB building to erase evidence against him. Michael seizes the chance to wipe his own data, freeing himself from Steve’s control.

After making up with his family, Michael plans a huge final heist with Trevor, Franklin, and Lester: robbing the Union Depository’s gold bullion reserve. But Trevor makes a shocking discovery – Brad wasn't imprisoned; he was killed in the Ludendorff heist and buried in Michael's "grave." Realizing the original heist was a setup and he was meant to die instead of Brad, Trevor feels utterly betrayed. He abandons Michael during a shootout with Cheng's henchmen. Franklin rescues Michael, but Trevor's anger creates huge tension within the group, threatening their plans. Amidst all this, Steve tries to arrest Michael, leading to a Mexican standoff between the FIB, IAA, and Merryweather. Steve escapes, but Trevor saves Michael and Dave, believing only he has the right to kill Michael.

The Union Depository heist is successful. Afterward, Franklin faces a tough choice. Steve and Dave argue that Trevor is a liability, while Devin, still seeking revenge, wants Michael dead. Franklin has three options: kill Trevor, kill Michael, or try a "suicide mission" to save both. If Franklin kills either man, his relationship with the survivor ends, and he goes back to his old life. However, if he chooses the third option, the trio, aided by Lamar and Lester, fight off attacks from the FIB and Merryweather. They then go on to kill Cheng, Stretch, Steve, and Devin. Michael and Trevor reconcile, and while the three protagonists stop working together, they remain friends.

This movie really explores the idea that you can't truly escape your past, no matter how hard you try.
